1. Share your equipment details
Have the manufacturer, model, serial number, operating conditions and reason for repair ready, along with your company and contact information.
2. Prepare equipment for service
Complete the Customer Equipment Form, also called the Clean Equipment Form, before sending equipment. It records the product handled, cleaning and draining status, and applicable Safety Data Sheet (SDS) information.
